On June 16, 1995 Gettysburg Railroad 4-6-2 #1278 had a minor boiler explosion severely injuring the crew. An investigation from the national transportation safety board determined the cause was poor training and maintenance. This video was produced shortly after as a training video for those operating steam locomotives.

Clearly an old regulations video, eg "at least one water glass and three gauge cocks should be operating." And, "there should be at least one 'working' steam gauge."
