Looking properly CPR, somewhere in eastern Pennsylvania, undated. Bethlehem?

Yes, that's Bethlehem Engine Terminal, and the date is most likely 1967.

Any idea when (and why) the Elesco coil type feedwater heater was removed from #1286 and replaced with an injector???

The feedwater heater on 1286 and the exhaust injector on 1238 were removed by Red Clay Valley in early 1969, after they bought the engines from George Hart.
