I do not recall which board had a discussion of how a switch that had the narrow gauge leave the standard gauge. I located a transition track used on a wye or a reverse loop and scanned it.
To make a switch out of it, the narrow gauge rails would cross the standard gauge rail with standard frogs.

I believe the rail that takes the train from one side of the track to the other is called a "draw rail", and as you stated they have to get the train from the "wrong" side to the correct side of the track when a turnout is needed on the "wrong" side (whichever side that may be) on a given three-rail track.
