Form 4 Question

I must be overlooking it, but can someone point out to me where you put the MAWP for the boiler on the new Form 4?

Look on the very last page which has the lines for the signatures of the person who performed the calculations, the locomotive owner and locomotive operator. This is after the alteration and waiver record. There is a paragraph where you fill in the locomotive initial and number and the MAWP.
